hi Michael,
yes I thought carefully about this before I reorganized because you  
cannot seem to print the whole thing at once.  I decided that  
hierarchal  chapter list was better.  I couldn't find anything in the  
manual ;)

  agreed.  That is why I started a new copy of the documentation  
rather than modifying the old.  I will rename that as 2.2 documentation
