+= list label doesn't work with EBNF operators

Martin Fowler found out that

w += WORD+

yields a null $w. The precedence is wrong on the += I think. (w += WORD)+ seems ok.

Made += and = bind tighter than EBNF stuff. Had to alter tree construction in antlr.g
